We see patients on an appointment basis and we request that you call
our office at 317.621.7120 in advance so that we can reserve time for
you. When you call for an appointment, our staff will ask you the following questions:
- Reason for Your Call, Problem, or Concern
- Telephone Number Where You Can Be Reached
- Date of Birth
New patients should arrive 15 minutes before appointment time to fill out medical history and other necessary forms. If you are unable to arrive
early, it may be necessary to reschedule your appointment.
We try our best to stay on schedule, although emergencies sometimes arise. If we are significantly delayed, we will try to notify you beforehand. Please assist us by being on time for your appointment.
If you have any changes in insurance, address, or other information, please arrive early. Patients, who do so, assist in maintaining patient flow and provider schedules.
Occasionally, patients in the waiting room may be seen out of order if they are urgent. We ask your patience if you have to wait. We will show you the same concern when you are seen.
If you find that it is impossible for you to keep your appointment, we ask that you inform us of the cancellation at least 24 hours in advance. This courtesy allows us to be of service to other patients.
