Phone requests for refills of routine medication may be left on voice mail by calling our office and letting our office staff know that you need a prescription refill. With your request, please be prepared to leave the following information:

• Your Name
• Your Birth Date
• Prescription Name and Spelling
• Dosage
• Pharmacy Name
• Pharmacy Phone Number
• Phone Number Where You Can Be Reached

Then call your pharmacy to determine if your prescription has been phoned in by our office. Messages left after 12:00 p.m. are answered the next business day.
